WhoHasGear title card: Equipment Loan Form Template, free editable Word and plain-text form, with three steps labelled Issue, Return and Close

An equipment loan form records one borrower, every item and accessory issued, its visible condition and the agreed return date, then checks the return against the same rows. This free Word and plain-text form keeps a loan Open or Partially returned until every listed item and accessory has been returned, or any outstanding item or issue has a documented resolution approved by the responsible staff member. It is a record, not a contract.

Download the equipment loan form

Download the editable Word form or the identical-content plain-text form. Both include instructions, the blank loan details, issue table, borrower acknowledgement, return table and status section, plus the completed example below. No signup is required.

These are manual files, not an online form or software feature. Print one per loan or keep an editable copy per loan ID.

The full download content follows in the same order. The issue and return tables use the same line numbers, so every returned item is checked against what was actually issued.

Equipment Loan Form Template

  • WhoHasGear | Original manual recordkeeping aid | November 2, 2026
  • One borrower, one loan ID, every item and accessory listed at issue, and the return checked against the same rows. A loan stays Open or Partially returned until every listed item and accessory has been returned, or any outstanding item or issue has a documented resolution approved by the responsible staff member.
  • How to use: fill the loan header, list each identified item on its own issue row, record accessories with quantities, note visible condition at issue, then get the borrower and issuer to sign the acknowledgement. When gear comes back, complete the return row for each line before changing the loan status.
  • Give every loan a unique loan ID. Use one row per individually tracked item; group identical untracked items only with a quantity. List accessories in the same row with their own quantities so a missing charger or case is visible on return.
  • Describe visible condition only, such as scratches, dents or a missing cap. Do not record a conclusion that equipment works or is safe; follow your own organization's checks where required.
  • Use full dates and times. Keep earlier entries and add dated corrections with author and reason rather than overwriting the original issue record.
  • This is a record, not a contract. It contains no liability, insurance, replacement-charge or legal terms. Add those only if your organization has its own approved wording.
  • This original worksheet has not been tested by operators. The completed example is fictional, not a customer outcome.

Partially returnedSome items or accessories are back; at least one line has an outstanding item or issue with a named follow-up owner.
ClosedEvery issue row has a completed return row, and any outstanding item or issue has a documented resolution approved by the responsible staff member. A pending investigation cannot close a loan. Only staff close a loan.

Completed example: why the loan is not closed

  • Loan status: Partially returned.
  • Outstanding: line 1 camera charger. Follow-up owner Sam Ortiz; next review November 6, 2026.
  • Closed by / date: left blank until the charger is returned or the outcome is recorded.
  • The camera, microphone kit and tripod are physically back, but the loan stays open on paper because one listed accessory is not.

How to use the form: issue, acknowledge, return, reconcile, close

Issue: fill the loan details, then list each identified item on its own row with its asset ID, serial where relevant, accessories with quantities and visible condition. “Camera body, battery x2, charger x1, strap x1” is checkable later; “camera kit” is not.

Acknowledge: the borrower and issuer sign that the listed gear was received in the recorded condition, with the agreed return date and place. The wording is a plain record of the handover, not legal terms.

Return and reconcile: when gear comes back, complete a return row for every issue row. Count accessories against the issue row, note visible condition and who received it, and write any difference in the outstanding column with a follow-up owner.

Close: change the status to Closed only when every row is complete and any outstanding item or issue has a documented resolution approved by the responsible staff member. A follow-up still in progress does not close a loan, and a borrower handing back the main items does not close it either.

What if only part of the loan comes back?

Mark the loan Partially returned. Record what did come back on each return row and list exactly what is outstanding, such as “line 1 charger x1,” with a follow-up owner and a next review date. Keep the original issue rows unchanged.

If the item is found, add the return date and who received it, then close the loan. If it is confirmed missing, record that outcome and use the missing equipment report form to document the search and closure for that one item.

Which equipment form do you need?

Use this loan form when one borrower takes several identified items and you want issue and return on one document.

For a running sheet where many people sign gear out at one desk, use the equipment sign-out sheet generator. Before a loan is agreed, collect requests with the equipment reservation request form.

If a borrower wants more time, record it with the equipment loan renewal request form. If gear passes directly from one borrower to another, use the equipment handover form template. To compare a multi-part kit against its manifest on return, try the equipment kit return checker.

What sources informed this form?

Published institutional loan forms use the same building blocks. Nashville State Community College’s equipment loan form records the borrower, tag number, serial, item description, reason for loan, approximate return date and a return section with the date returned. Salisbury University’s form lists each item on a separate line with tag number, description and serial. The University of Alaska Fairbanks Disability Services form records item identification, a due date, and on return asks whether the equipment came back in its original state.

Those forms carry their own institutions’ terms, approvals and responsibilities; this template does not reproduce them. Form builders such as Jotform offer online equipment loan forms as another route.

Who this form fits — and its limits

Use it for a small team, school department, church, club or community group lending identified gear to one person at a time, such as cameras, audio kits, tools or laptops. An employee borrowing equipment temporarily can use it too; long-term device assignment and offboarding are different workflows.

It is not a contract or legal agreement. It contains no liability, insurance, replacement-charge or enforceability terms and is not legal advice. If your organization needs those, use its own approved wording.

When a paper loan form stops being enough

A form per loan works while one person issues gear and loans are occasional. It gets harder with repeated loans, several issuers, overdue items to chase, or nobody being sure who has an item right now, because the current picture lives across many sheets.

WhoHasGear is QR-based equipment checkout and reservation software for small teams.

This form is separate from the software and does not sync with it. When you want checkouts, due dates and custody history in one place, see WhoHasGear equipment checkout software and its pricing.
