WhoHasGear missing equipment report: identify the item, record custody and searches, verify recovery; free Word and text template

A missing equipment report form records one missing item, its last confirmed sighting, custody evidence, searches and verified resolution. Download the editable Word template or copy the complete form below; keep the original report when new facts arrive.

Get the editable Word template or complete plain-text form. Both include the seven blocks and the same fictional example shown below. No signup is required.

The Word file uses editable text and expandable custody and search tables, not an image of a form. Add rows as the search develops. The text version is suitable for copying into your own private document.

Use this as an internal incident record, not a police report, insurance claim, disciplinary finding or financial authorization. It does not determine blame, certify safety or replace your organization’s separate procedures.

When should you open a missing-item report?

Open a report when one identified item cannot be located after the discrepancy is noticed. Record what is known now; do not wait for a complete explanation or turn an uncertain recollection into a fact.

The inventory audit checklist covers a broader count and reconciliation. This form follows one unresolved item. An equipment handover form records a transfer, rather than reconstructing an uncertain one.

How do you record evidence without assigning blame?

Write the source beside each custody statement and keep unknown intervals visible. A checkout entry establishes what the record says; a confirmed sighting establishes that someone observed the item at a particular time and place.

Copy last-seen and last-used times separately. Someone may have used the projector at 08:00 and seen it on a cart at 08:15. Neither fact proves who moved it afterward.

A negative search rules out only its stated scope at that time. Write “Hall A cupboard 2, all shelves, checked at 09:25,” rather than “building searched.” Keep planned checks marked Not checked.

Missing Equipment Report Form (Free Word Template)

  • WhoHasGear | Manual internal incident record | October 26, 2026
  • Use one report for one identified missing item. This document does not submit a report or update software. No account is required.
  • Record known facts, label estimates and write Unknown where necessary. Include the time zone with every timestamp. A system-recorded borrower is not proof of physical custody or responsibility.
  • Keep completed reports private. Reference private checkout, handover and photo records instead of copying personal contact details or sensitive logs into this form.
  • Add rows to the custody and search tables as needed. In Word, use Insert Rows Below or press Tab in the final cell. In plain text, duplicate the row fields. Check print preview after editing.
  • Retain the original report. Append dated updates and corrections with author and evidence; do not erase earlier statements. Recovery requires identity and location verification before closure.
  • Internal recordkeeping only: not a police report, insurance claim, blame or liability finding, financial authorization or safety certification. Follow separate applicable procedures outside this form.
  • These are proposed fields, not an institutional standard. WhoHasGear has not field-tested this template with an operator team.

  • Original report reference retained: __________ | Appended update / correction references: __________
  • Use Resolved-found only after identity, current custody/location and record-update status are documented and reviewed. If verification is pending, retain Found awaiting verification.
  • Closed-unresolved records an authorized administrative decision, not a recovery or a write-off. Preserve the missing-item history and state what would reopen the search. Recovery does not certify readiness for use.

Fictional completed example: projector found after a custody gap

  • All people, assets, records and events below are fictional. All timestamps are October 26, 2026, EDT (UTC-04:00). This is not a customer result.
  • 1. Report identity: MIS-026, original version 1, Northbank media team. Morgan Ellis discovers the empty projector shelf at 09:00 and reports it at 09:10. Morgan owns the search; next review 12:00. Initial status Open/searching; private reference CASE-MIS-026.
  • 2. Exact identity: projector PRJ-021, fictional serial NB-P021, Northbank Demo P1, blue label beside the lens. Standalone projector, not the carry case. Normal storage Hall A, cupboard 2; inventory INV-021; photo PHOTO-021.
  • 3. Last sighting and use: Alex Reed confirms using PRJ-021 in Hall B at 08:00 and seeing its label on the Hall B cart at 08:15, statement SIGHT-01. Both times are exact for this example. System record LOAN-078 names Alex at 07:45; that entry alone does not establish later possession. Latest reported physical holder is Alex during use at 08:00, based on Alex's account; independent verification is not documented. Movement after 08:15 is initially unknown.

4. Custody: checkout, 07:45Store to Alex; recorded destination Hall BLOAN-078; entered by Morgan at 07:45System-recorded only; not an independent sighting
Use 08:00; sighting 08:15Alex; Hall B, then Hall B cartSIGHT-01; Alex statement entered by Morgan at 09:20First-person account of use and sighting; independently unverified. No confirmed handoff afterward.

Recovery sighting, 10:25Jamie Park; Hall C, cupboard 1FIND-01; entered by Morgan at 10:30Found awaiting verification; original movement still unexplained

Fictional example continued: searches and verified closure

  • 5. Search log: outcomes record what was actually checked, not what the team hoped to check.
Search targetCheck detailsOutcome and evidenceNext action
Hall A cupboard 2, all shelvesMorgan, 09:25Checked - not found; SEARCH-01 records empty assigned shelf and other shelves checkedRuled out at 09:25 only; Morgan to revisit if new evidence emerges
Checkout and transfer entries for PRJ-021Morgan, 09:30Checked - not found; no later transfer entry, LOG-CHECK-01Morgan to ask for physical sightings by 12:00; no record is not proof of no movement
Hall C cupboard 1Not checked at 09:35; Jamie assignedNot checked; PLAN-01Jamie to check by 11:00
Hall C cupboard 1, repeat targetJamie, 10:25Checked - found; blue-labelled projector, FIND-01Morgan to verify tag and serial before closing

6. Operational follow-up: Hall A presentation at 13:00 affected. Substitute PRJ-022 held provisionally, arrangement BOOK-02. Morgan notifies the presentation team privately at 09:40, MSG-01; acknowledgement received 09:45. At 10:30 Morgan appends Found awaiting verification, UPDATE-01. Next action: identity/location check by Morgan at 11:00.

7. Resolution: Jamie found the projector in Hall C cupboard 1 at 10:25. Morgan independently matches PRJ-021 and serial NB-P021 against INV-021 at 10:40, evidence VERIFY-01. Morgan confirms receipt into Hall A cupboard 2 at 10:50, RECEIPT-01; current custodian Morgan.

At 10:55 Morgan appends the actual receipt and verified location to the manual custody ledger, UPDATE-02. The old LOAN-078 entry is retained and cross-referenced; no return time is backdated. Record-update status Completed. At 11:00 reviewer Dana Lee, authority TEAM-COORD-01, closes MIS-026 as Resolved-found, CLOSE-01.

CASE-MIS-026 version 1, SIGHT-01 and all updates remain attached. The 08:15–10:25 movement gap remains unexplained, not assigned to Alex. Morgan owns a process follow-up on October 27 at 09:00 EDT to check cart handoff recording. Reopen if identifier verification is contradicted. Recovery is not a readiness-for-use certification.

What supports this form, and what is our own design?

Institutional forms support recording item identity, last-known facts and completed searches. Our custody chronology, explicit search outcomes and detailed verification block are design additions, not a universal institutional standard.

UTA’s initial form requests an asset tag, serial, last-known location, last-seen time and custodian. Its final form requests locations checked and relevant record reviews. Both documents are dated December 2025; we do not import their deadlines or administrative determinations.

ETSU separates last seen from last used and asks what steps were taken to locate equipment. No revision date appeared in the extracted text. UTSA’s reinstatement landing page was revised January 2024; its hosted questionnaire fields were not verified.

Virginia Tech’s inventory responsibilities page, revised February 24, 2026, supports checking readable barcodes and location evidence. It is an inventory procedure, not a missing-item closure form. All sources below were retrieved October 26, 2026.

What does the community evidence show?

One r/sysadmin poster described expensive tools going missing and wanted self-service checkout. The poster also worried about users assigning items to other people. That is a reason to distinguish a recorded name from confirmed custody, not evidence of wrongdoing.

We retrieved “Looking for a simple self-service check-in/check-out system for employees to borrow tools” on October 26, 2026. Reddit displayed “5y ago”; the exact posting date was not independently verified. One accessible first-person account does not establish how common the problem is or prove this template works.

Who is this template for, and where does software fit?

This form suits a small-team coordinator documenting one missing item and a manual search. It does not suit legal reporting, insurance decisions, financial write-offs or safety approvals. We have not field-tested it with an operator team.

WhoHasGear is QR-based equipment checkout and reservation software for small teams.

Use checkout, return and item history to maintain routine equipment records. Explore the equipment checkout hub or WhoHasGear demo to understand that workflow.

This downloadable incident form remains separate from WhoHasGear. It does not provide native incident management, automatic searches, recovery, notifications or case closure. If a recovered item needs repair follow-up, use a separate equipment repair tracking log; recovery alone does not establish readiness for use.
