Team Equipment Checkout Sheet Generator (Free)
Set up your roster, list the gear you issue, and get a printable packet: one issue sheet per player, a master inventory, a numbered checkout and return log, and QR labels for every item number. Download the log as CSV and it opens straight in a spreadsheet. Everything runs in your browser — no signup, no upload, and your roster never leaves this page.
Roster (3 players with a name, max 100)
One line per player, comma or tab separated: name, number, position, size. Pasted from a spreadsheet works too — copy the four columns and paste them straight in. You can also type rows by hand below.
| # | Player name | Number | Position | Size | |

Printing tip: the date-out, condition and returned columns print blank on purpose — fill them in with a pen at hand-out and again at return. The CSV leaves the same columns empty so your spreadsheet can take the entries instead.

MAKE THE SHEET DO SOMETHING
A sheet tells you who should have it. WhoHasGear tells you who actually does.
Paper works until two people want the same helmet, or a player leaves the team with three items nobody listed. WhoHasGear keeps the same roster and gear list as a live system: scan a QR label with any phone, the borrower types their name, and the whole team sees what is out, what is overdue, and who had it last. No app for parents, no logins to hand out.
- Start from the roster you just typed
- Borrowers scan a label, no app needed
- Overdue items flagged on your dashboard
- Full custody history per item
No credit card. The generator above stays free either way.
Key takeaways
- A checkout sheet answers three questions: who has it, in what size and condition, and what has not come back.
- Number every issued unit, not every product line, so a loose helmet traces back to one player.
- Record the condition at hand-out. It is the cheapest protection against a damage argument in November.
- One signed sheet per player beats one master list with initials in the wrong column.
- Give every player a due date, even if it is just “end of season”.
- Count the gear on a schedule, mid-season and at the end, or the paper drifts within a month.
- All of this works on paper today. Scanning is an upgrade, not a prerequisite.
What belongs on a team checkout sheet
A checkout sheet has one job: say who has what, in what size, and what is still missing. Every line on it should serve that job.
Keep the columns boring and fillable by a volunteer standing in a hallway. Anything that needs explaining gets left blank.
- Player name and number — two identifiers, because names repeat and numbers get borrowed.
- Squad, team or position — so you know who to ask when gear moves between groups.
- Item and size — “helmet, L” is a fact; “helmet” is a guess.
- Item number — the number printed on the sticker or QR label for that unit.
- Quantity — two jerseys issued is not one jersey.
- Date out and due back — a date is what turns “some time in October” into overdue.
- Condition out and condition back — plus one short notes line for missing pieces.
- Two signatures — the person issuing the gear and the player or parent receiving it.
Number the units, not the products
A product line is “helmet, size L”. A unit is one physical helmet sitting in one locker. When you only number product lines, two helmets of the same size become impossible to tell apart.
The generator above numbers units for you as you type. Every player and every item gets its own ID, and the numbers appear in the checkout log and on the label sheet.
Print the labels on plain Avery sheets using the same free asset tag generator layout this tool follows. Stick one on the helmet shell, not the face mask, and one on the gear bag handle.
How to run hand-out day
Hand-out day fails when it becomes a queue. Set up two tables, one for hard goods and one for soft goods, so nobody waits behind a helmet fitting to collect a jersey.
- Print the packet the night before — one issue sheet per player, one log, and a master inventory for the clipboard.
- Sort gear by size before anyone arrives — the sizes on the sheet should match the piles on the table.
- Write the condition while the player is standing there — scratches and cracked shells are easy to note, awkward to recall.
- Get both signatures before they walk away — a sheet signed later tends to stay unsigned.
- Keep the signed sheets in the equipment room — a binder at home is a binder nobody can check at practice.
- Photograph anything already damaged — one photo per item turns a debate into a lookup.
Closing the season without a scavenger hunt
The return pile never matches the issue sheets on the first pass. That is normal, and it is exactly what the sheets are for.
Work the numbered log top to bottom, ticking the returned column for each unit. Whatever is left unticked is your follow-up list, and it names a person instead of a missing item.
- Collect by squad or team — smaller groups make missing items obvious sooner.
- Re-check condition at return — write the new damage in the condition-back column.
- Set aside anything damaged — one box labelled “repair or replace” beats a note nobody reads.
- Count what is in the room — our inventory audit checklist walks through the end-of-season count step by step.
- Refresh the master numbers — use the free inventory log generator to rebuild the master inventory from what actually came back.
What this tool does not do
This generator makes the paperwork. It does not store anything after you close the tab, and it cannot remind anyone of anything.
- No saved files on our side — reload the page and you start from the example again, so download the CSV before you leave.
- No live status — the sheet only knows what was written on it, not what changed yesterday at practice.
- No booking or availability — if two squads want the same 20 helmets, the sheet will happily list both.
- No reminders or history — overdue chasing and “who had it last” live in a tracking system, not on a page of paper.
When the clipboard stops keeping up, that is the moment WhoHasGear takes over. You can start free with the roster you already typed here, or watch the scan flow first.
